Most placements for Health Sciences programs require the following:
If you are a returning student, bring the mandatory medical and non-medical form that you took to the Health Centre in previous semesters to obtain a passport. When you submitted your form to the Health Centre previously, you were advised to ensure you had a copy for your records prior to submitting the form to the Health Centre. If you did not make a copy, you will need to go to the Health Centre and retrieve a copy prior to your pre-requisit screening appointment.
